Fees are subject to change without notice. Please inquire prior to your visit. One unit = ≤ 15 minutes.
Phone Consultation (a brief “meet and greet” prior to initial consultation):  Free ≤ 15 minutes in duration.

INDIRECT CARE:  CORRESPONDENCE, COMMUNICATION, AND CARE PLANNING
$50.00 per unit. Indirect care may be provided in person, via telephone, videoconference, and by all other electronic means. Indirect care consists of treatment planning, care coordination, consultation with other health care providers, referral to other health care providers, travel, completion of forms for the benefit of the patient or on the behalf of the patient, and all other indirect support and care provided for the benefit of the patient or on behalf of the patient.

Laboratory fees may be paid with HSA or FSA cards and a three month interest free payment plan is available for all patients. We partner with Rupa Health for lab testing. Their policies regarding use of private insurance for lab testing can be found here, and they will provide assistance with superbills upon request if you would like to seek reimbursement directly from your insurance plan. 

Cost of Services  

When reviewing the cost of services, keep in mind that cost is relative to time spent on individual case review, evaluation, diagnosis, and treatment planning; a provider’s level of education and years in practice, licensure and board certifications, specialty designations, additional certifications, and areas of expertise.

Payment

Payment in full is expected at the time of service. We accept VISA, Mastercard, Discover Card, American Express, Flexible Spending Account (FSA) cards, and Health Savings Account (HSA) cards. You will be charged $50.00 for any form of payment not honored by the bank.

Secure electronic payment may be rendered via PayPal or Square by using the link provided in your appointment confirmation email. You may also render payment:

            • Via mobile pay (Apple Pay, Samsung Pay, Google Pay, and Fitbit Pay)
            • With a contactless proximity pay enabled credit card
            • With a chip enabled credit card or debit card
            • With a magnetic stripe enabled credit card or debit card
            • Via cash or check

PRIVATE OR GROUP HEALTH INSURANCE:  You are expected to pay for services in full at the time the care is provided. We are not, by choice, “in-network” with any insurance plan. Please contact your health insurance company to determine if your plan offers benefits. We will provide you with a superbill upon request if your insurance plan offers “out-of-network” coverage and you would like to seek reimbursement directly from your insurance plan. A superbill is an itemized list of services rendered, including diagnosis codes and medical billing codes. Please contact your health insurance company or refer to your policy for inquiries about benefits and coverage process.

OTHER INSURANCE:  We do not accept Workers Compensation cases or motor vehicle accident insurance cases. We do not submit claims to insurance companies or 3rd party payers.

Cancellation and Late Arrival Policy

We ask for a 24-hour cancellation notice in order to accommodate other patients, and to provide you with the best possible results in a manner that is timely and cost effective for you. If you cancel or reschedule with less than 24 hours notice, you may be charged your entire appointment fee. If you cancel or miss two appointments in a row, you may be discharged from our services. If you return and are readmitted for care, you may be charged the initial visit fee for your first visit. You may be asked to reschedule your appointment if you arrive more than 15 minutes after your scheduled appointment time.

This policy is enforced strictly for a number of reasons. For instance, given proper notice we can fill the vacancy with another patient who is in need of care, but may have had to wait due to a full schedule. We often have a waiting list and a missed appointment means a waste of valuable care. We receive messages 24 hours a day, seven days a week. All messages are time and date stamped.

Telephone Policy

We strive to return all phone calls before the end of each business day. In order to accomplish this, we must limit our callbacks to five minutes. If your concerns cannot be addressed in that time frame, an office or telemedicine visit may be scheduled in order to best meet your needs.

Are you waitlisted for an earlier appointment?

Patients who have completed and submitted the paperwork and who are waitlisted for an earlier appointment will be given priority for earlier appointments that become available. Completing and submitting your paperwork ahead of time is evidence of investment in your own health and serves you best by allowing us adequate time for thorough review before your visit. Please enroll in or access your secure patient portal below and submit your forms as soon as you can, and no later than 24 hours before your appointment. We look forward to seeing you soon!
